Michigan Health Council (MHC) is pleased to support Beaumont Health in their new initiative “Advance Care Planning Starts Today: Respecting Choices at Beaumont.” In 2019, MHC and Beaumont Health applied for and received a Healthy Aging Grant from the Michigan Health Endowment Fund. This collaborative project between MHC and Beaumont will help expand the implementation of the Respecting Choices® evidence-based Advance Care Planning (ACP) model outside of the acute care setting in order to reach older adults in Southeast Michigan and beyond.
Michigan Health Council will support the ACP expansion efforts by providing technology to deliver ongoing mentorship opportunities and creating continuing education materials. These course offerings will be accessible across the state. They will set the standard for ACP best practices and provide a method for tracking competency and collecting quality metrics for ACP facilitators. Additionally, MHC will provide statewide connections and infrastructure to assist Beaumont’s Older Adult and Caregiver Retreat for the community at large.
ACP Michigan, a program of Michigan Health Council, provides advance care planning information and resources. To learn more, please visit acpmich.org. Or, learn more about Respecting Choices at Beaumont at bit.ly/respectingchoices.
